Adult Foster Care (AFC)

The AFC program provides personalized support and care coordination for eligible individuals living at home with family members or other qualified caregivers.

Services May Include

  • Assistance with personal care
  • Nursing oversight
  • Care management
  • Emotional and social support
  • Ongoing monitoring and coordination

Program Eligibility for Member

  • Must have MassHealth Standard
  • Require assistance with at least 1 ADL (Activity of Daily Living)
  • Must be 16 years or older
  • Must have a family member residing in the same household who can serve as the caregiver. The caregiver cannot be a spouse. If the member is a minor or has a legal guardian, the caregiver cannot be the parent or legal guardian.

Program Eligibility for Caregiver

  • May be a family member, friend, or other trusted individual residing with the member
  • Caregiver must be 18 years or older
  • Must complete all required training and orientation
  • Must successfully pass all required compliance and pre-employment screenings
  • Must provide proof of TB screening and medical clearance/physical fitness approval from a physician

Program Benefits

Group Adult Foster Care (GAFC)

We proudly provide GAFC services under MassHealth for eligible individuals who require assistance with daily living activities while remaining at home or in a community setting.

Our GAFC Services Focus On

  • Personal care assistance
  • Medication reminders
  • Health monitoring
  • Care coordination
  • Support with activities of daily living

Program Eligibility for Member

  • Must be 22 years or older
  • Live in subsidized housing
  • Have a primary care physician
  • Require assistance with at least 2 ADL (Activity of Daily Living)
  • Meets MassHealth income requirements

Private Pay Services

We also offer flexible private pay home care services for individuals and families seeking personalized in-home support outside of MassHealth programs.
